Oxygen is an established acute treatment for cluster headaches, but obtaining equipment, refills, and insurance approval can be challenging. This guide explains cylinder oxygen, stationary concentrators, and questions to ask about home filling.

What oxygen setup is recommended?
Clinical guidelines recommend high flow oxygen through a nonrebreather mask for acute attacks; the strongest established evidence is for medical cylinder oxygen. Your clinician should prescribe the oxygen source, flow, mask, and treatment duration. A standard nasal cannula is not an equivalent setup.
Help getting oxygen through insurance or a supplier
- Ask your treating clinician for a prescription that clearly specifies cluster headache, oxygen delivery equipment, flow, and duration.
- Ask your insurer which suppliers participate, what prior authorization is needed, and your full out of pocket cost.
- If denied, request the written reason and appeal instructions. Ask the prescriber to submit diagnosis documentation and a medical necessity letter.
- Ask suppliers for itemized prices for cylinders, refills, rental, delivery, regulators, and masks. Compare equivalent cylinder capacities.
You deserve a workable treatment plan and clear equipment information. Your treating clinician can help you evaluate oxygen options and an individual treatment plan.
HSA cards accepted
Bradshaw Oxygen accepts Health Savings Account (HSA) cards. HSA funds can be used for qualified medical expenses. Keep your purchase receipt and any supporting prescription or medical necessity documentation. Card acceptance does not by itself determine whether a particular purchase qualifies; confirm eligibility for your intended medical use with your HSA administrator or tax adviser.
Insurance coverage and access
Coverage varies by insurer, plan, documentation, and supplier. Some patients encounter denials or difficulty finding a supplier, but oxygen for cluster headaches is not universally excluded. Aetna recognizes qualifying cluster headache oxygen treatment; Medicare coverage decisions are handled by regional Medicare Administrative Contractors. Ask your insurer about coverage, prior authorization, participating suppliers, and appeals before paying privately. Bradshaw Oxygen does not work with insurance companies or bill insurance. Purchases are paid directly to Bradshaw Oxygen. HSA cards are accepted for eligible medical purchases.
How does HomeFill work?
A compatible oxygen concentrator supplies oxygen to a HomeFill compressor, which compresses the gas into compatible cylinders for later use. Filling happens between uses; cylinder delivery flow is controlled by the cylinder regulator, not the concentrator's 5 or 10 LPM rating. HomeFill compresses the concentrator's oxygen without increasing its purity.
Can a 10 LPM concentrator be used directly?
Some patients report relief from direct 10 LPM stationary concentrator use, while others report inadequate relief. These reports do not establish that it will work for you. Flow, oxygen concentration, mask fit, and individual response all matter; direct concentrator use can provide relief for some patients. Direct use as a backup should be agreed with your treating clinician, with a separate rescue plan if it does not relieve an attack.
For the Invacare Platinum 10, manufacturer instructions limit direct patient flow to 5 LPM while HomeFill is filling. Filling must stop before direct use above that limit.
Evidence for concentrator oxygen
Concentrator oxygen can help some people with cluster headaches. A 2024 randomized crossover study used two 5 LPM EverFlo concentrators together to supply 10 LPM through a nonrebreather mask. Substantial pain reduction at 15 minutes occurred in 31.7% of oxygen treated attacks versus 12.9% with oral zolmitriptan. The study provides clinical evidence of benefit from concentrator oxygen. It did not test our Invacare HomeFill configuration or compare it directly with medical cylinder oxygen; effectiveness of this specific setup has not been established.
Bradshaw Oxygen reports typical measured output of 94 to 96% for its tested units. Actual concentration depends on the unit, flow setting, and operating conditions; this is not a guarantee for every unit at every setting. Concentrator oxygen is not automatically ineffective because of its concentration: published research and patient reports describe relief in some users. Results vary, and measured equipment output does not establish how well a particular person will respond.

Compatible with larger oxygen tanks
The Bradshaw Oxygen HomeFill setup can accommodate E cylinders and larger oxygen cylinders with the appropriate filling whip, valve connection, and rated pressure. You are not limited to the small cylinder pictured. Contact us to confirm compatibility with your particular tank. Customers supply their own cylinder.
For breathing use, the cylinder must be suitable and clean for medical oxygen service, with known prior contents and current required testing. An industrial connection alone does not establish suitability for inhalation. Cylinders, delivery regulators, and masks are not included.
10 LPM HomeFill package under development
Our proposed package combines a reconditioned Invacare Platinum 10 LPM concentrator, a reconditioned HomeFill compressor, and a cylinder filling whip. The customer supplies their own compatible oxygen cylinder, including larger tank sizes, prescribed delivery regulator, and mask. Cylinder compatibility, filling pressure, and the complete configuration must be confirmed before ordering. A fitting that connects mechanically does not establish suitability for medical oxygen filling. The package is not represented as a clinically validated cluster headache treatment or a substitute for prescribed cylinder oxygen.
Questions to resolve before choosing a cylinder
- What cylinder valve and filling connection are supported by the selected whip?
- Is the cylinder approved for oxygen service, within its required inspection and test dates, and rated for the intended fill pressure?
- Does the full filling setup comply with the equipment instructions and requirements for medical use?
- Has your clinician agreed that the oxygen source and delivery setup are appropriate?
Do not refill supplier owned or rented cylinders without the supplier's permission. Keep oxygen equipment away from flames, smoking, oil, and grease. Follow the equipment manuals.
Compare 5 and 10 LPM options
Our HomeFill setups collection includes a budget 5 LPM cylinder filling configuration and a 10 LPM configuration. The 5 LPM option is for filling cylinders, not direct high flow cluster headache use.
Choose the components for your setup
Each setup uses one concentrator, one HomeFill compressor, and one filling whip. Components are purchased individually. The customer supplies a compatible oxygen cylinder, delivery regulator, and prescribed mask. A prescription is required for medical use.

The concentrator supplies HomeFill for cylinder filling. Its 5 LPM direct output is below recommended high flow cluster headache delivery.
10 LPM setup: cylinder filling and potential direct backup
Some patients benefit from direct 10 LPM concentrator oxygen; discuss backup use with your treating clinician. Stop HomeFill filling before using the Platinum 10 directly above 5 LPM.
